ISMP Updates Guidelines to Address Medication Safety Risks
The Institute for Safe Medication Practices (ISMP) has updated guidelines in an effort to improve how medication information is communicated through electronic health records (EHRs). The revised “
Guidelines for Safe Electronic Communication of Medication Information,” last updated in 2019, reflects challenges with health information technology and aims to reduce medication errors in inpatient and outpatient settings
The new guidelines include recommendations for expressing drug names clearly, including the use of tall man lettering to help distinguish medications that look alike. In addition, the guidelines recommend improving drug search and selection errors and work to ensure essential information such as dosage route and form remains visible. The recommendations apply across all types of health IT systems used to manage medications, including EHRs, pharmacy platforms, e-prescribing systems and smart infusion pumps.
FDA Commissioner Announces Resolution of IV Saline Solutions Shortage
Today, Food and Drug Administration (FDA) Commissioner Marty Makary, M.D., M.P.H, issued a
statement to announce the end of the nationwide shortage of sodium chloride 0.9% injection products, a form of intravenous (IV) saline. These products will be removed from the FDA list of Current Shortages in the Drug Shortage Database on the FDA website. The FDA said it generally recommends hospitals, health systems, and clinics use the FDA-approved drug when available. For other IV fluids still in shortage, the FDA said it “is working closely with manufacturers and will continue to monitor the supply to help ensure patients have access to the medicines they need.”
CDC COCA Call: Clinician Update on U.S. Measles Cases and Outbreaks
This year, record-breaking numbers of U.S. measles cases and outbreaks have been reported, despite measles being declared eliminated in 2000. While the risk of widespread measles remains low for the general population due to high population-level immunity, measles importations continue to occur.
During a Centers for Disease Control and Prevention
Clinician Outreach and Communication Activity (COCA) Call, presenters will discuss the current epidemiology of measles in the United States and address the most common questions from clinicians about preventing, identifying, and testing for measles. Presenters will also summarize measles, mumps, and rubella (MMR) vaccination recommendations and considerations.
